Gene deletion forces national antigen switching decisions
Falciparum parasites lacking the histidine-rich protein 2 genes return negative results on tests that detect only that antigen, and surveys across the Horn of Africa found false negative rates reaching roughly 12% in affected populations. Guidance sets a 5% threshold at which a country should change test type, and several have. Deleted strains escape detection and therefore treatment, which gives them a selective advantage and makes the spread self-reinforcing rather than incidental. Surveillance coverage remains patchy across much of the affected transmission belt, which means many programmes are switching on precaution rather than on local evidence they can actually obtain.
